You will work as part of a multidisciplinary team delivering holistic care to adult inpatients on a 28-bed medical ward.

You will manage a defined caseload, complete assessments and deliver evidence-based interventions in service users homes and clinics.

The post holder will work within a busy outpatient department providing assessment, treatment and rehabilitation programmes.

You will assist registered nurses with observations, hygiene needs and mobility of post-operative patients.

The role includes chronic disease management, childhood immunisations and treatment room duties.

Work closely with medical and nursing teams to optimise medicines use and ensure patient safety.
